No deductible, no coinsurance, no copays — HPG pays a fixed cash benefit directly for covered services, on top of any other coverage the client has. Benefits are highest when the client uses a UnitedHealthcare Choice Plus network provider (1.8M+ providers, 7,000+ hospitals, 58% average discount savings) but the plan pays regardless of provider choice. Sold in three tiers: Select 2000, Preferred 4000, and Premier 5000.

A limited-benefit term life policy with an optional Critical Illness accelerated benefit rider — a simple, affordable add-on conversation for clients who don't have any life coverage in place. Not major medical or comprehensive health insurance.
| Feature | Detail |
|---|---|
| Policy Term | 10-year term (issue ages 18–59) OR 20-year term (issue ages 18–49) — client picks one |
| Term Life Benefit Amount (choose one level) | $30,000 | $25,000 or $50,000 | $50,000 or $100,000 | $75,000 or $150,000 | $100,000 or $200,000 |
| Optional Critical Illness Max Lifetime Accelerated Benefit | $15,000 | $25,000 | $50,000 | $75,000 | $100,000 (tiered with the Term Life amount selected) |
How it works: any Critical Illness benefit paid is subtracted from the remaining Term Life death benefit — the client gets cash now for a qualifying diagnosis, at the cost of a lower payout later. A 30-day waiting period applies to the Critical Illness rider except as noted below.
| Qualifying Illness | % of Critical Illness Max Benefit Paid |
|---|---|
| Heart Attack or Stroke | 100% |
| Life-Threatening Cancer — diagnosed >90 days after effective date | 100% |
| Life-Threatening Cancer — diagnosed within the 90-day waiting period | 10% |
| Carcinoma in Situ | 25% |
| Coronary Artery Bypass Graft | 25% |
| Coma / Major Organ Transplant / Paralysis / Renal (Kidney) Failure | 100% |
| Loss of Hearing, Speech, or Vision | 100% |
